Coordination of Supports
Support Coordination is an NDIS capacity-building support designed to help participants understand, navigate, and make the most of their NDIS plan and broader support systems.
At Recovery Together, our Support Coordinators work alongside individuals to build confidence, increase independence, and create supports that genuinely align with a person’s goals, needs, identity, and lived experience.
What to Expect from our Support Coordinators and Psychosocial Recovery Coaches
At Recovery Together, we understand that navigating the NDIS can be complex. That's why we provide two specialised coordination services designed to help participants maximise their NDIS plans, achieve their goals, and build greater independence.
While both roles help participants coordinate supports and services, they each have a distinct purpose and focus.
Support Coordination
Support Coordination helps participants understand and implement their NDIS plans while building the skills and confidence needed to manage supports independently over time.
Our Support Coordinators work alongside participants, families, carers, service providers, and community organisations to ensure supports are working effectively and meeting individual goals.
What Our Support Coordinators Do
Our Support Coordinators help participants to:

Understand their NDIS plan and funding budgets

Implement supports and services included in their plan

Connect with NDIS providers and community supports

Coordinate mainstream, community, health, and government services

Build capacity to make informed decisions and manage supports independently

Monitor progress towards goals

Address barriers that may impact outcomes

Prepare reports and recommendations for NDIS plan reviews

Advocate for participants when required
Psychosocial Recovery Coaching
Psychosocial Recovery Coaching is specifically designed for participants living with psychosocial disability arising from mental health conditions.
A Recovery Coach focuses on supporting participants through their personal recovery journey by building resilience, increasing independence, and strengthening their connection to community and support networks.
Recovery Coaching is a capacity-building support that focuses on long-term recovery outcomes rather than simply coordinating services.
What Our Recovery Coaches Do
Our Recovery Coaches work collaboratively with participants and those important in their lives to:

Develop strategies to manage challenges and setbacks

Working with a Recovery Coach
Recovery Coaches spend time getting to know participants, their goals, strengths, support networks, and recovery journey.
They help participants:

Understand the NDIS and available supports

Learn about evidence-based mental health supports

Access community, health, and mental health services

Build skills for greater independence

Maintain engagement with services during challenging periods

Develop recovery plans and strategies for future success
Many participants find value in working with Recovery Coaches who have lived experience of mental health recovery, providing additional understanding and insight into the recovery journey.

Getting Started with Support Coordination and/or Psychosocial Recovery Coaching
Recovery Together currently accepts referrals for participants who are Plan Managed and have approved funding for Support Coordination and/or Psychosocial Recovery Coaching within their NDIS plan.
Before Making a Referral
To ensure a smooth intake process and timely commencement of services, referrals should include:

A copy of the participant’s current NDIS plan

Current Plan Manager details

Confirmation of available Support Coordination and/or Recovery Coaching funding

Participant contact details
-
Relevant information regarding goals, support needs, risks, and existing supports

Details of any key stakeholders involved in the participant’s support network
Providing complete referral information helps us assess suitability, allocate the most appropriate team member, and minimise delays in service commencement.

Capacity Building
Recovery Together’s Capacity Building supports help individuals develop the confidence, practical skills, and supports needed to navigate everyday life, housing, routines, decision-making, budgeting, and major life transitions with greater independence and wellbeing.
Our trauma-informed and person-centred approach works alongside individuals to build sustainable supports that feel safe, empowering, and tailored to their unique goals, strengths, and lived experiences.

Support Workers
Recovery Together has partnered with a number of trusted and values-aligned Core Support providers who assist participants to access their community, attend appointments, complete shopping and daily tasks, maintain their home and yard environments, engage in social and recreational activities, and build confidence within employment and workplace settings. Our network is qualified and experienced in building greater independence, inclusion, connection, and wellbeing while ensuring participants receive compassionate, person-centred, and trauma-informed support aligned with their goals, interests, and support needs.

This directory connects participants, families, carers, and referrers with trusted Recovery Together partners across plan management, support coordination, psychosocial recovery coaching, core supports, allied health, mental health, employment, and community participation services.
